New Client: Assurant (out of network, through Stratose), Effective 7/1/15

Effective July 2015, Assurant Employee Benefits* will have access to Participating Provider fee arrangements with Maverest Dental Network. Member ID Cards will not reflect the Maverest Logo. Please verify member eligibility and benefits by calling the phone number listed on member ID cards or the Assurant website. The Explanation of Benefits (EOB) for these claims will identify Maverest as the company used to determine the applicable fee schedule.

As part of this agreement, you can now see Assurant Employee Benefit members under the terms of your dental provider fee arrangement with Maverest. Please bill at your usual fees for all services.

If you also participate in a dental PPO network, which Assurant Employee Benefits offers to its insureds, you will continue to see Assurant Employee Benefits patients according to the terms of the contract you have with the applicable PPO network.
